A focus on assisting members with health conditions through personalized goal setting and collaboration among the care team. Our specialized programs, developed exclusively for OhioHealthy Members, connect them to resources and professionals across the health and wellness continuum, at no additional cost. We provide members pathways to better health including programs for: 

  1. Diabetes
  2. Weight Management
  3. Chronic Obstructive Pulmonary Disease (COPD)
  4. Chronic Kidney Disease (CKD)
  5. Cardiovascular (Hypertension, High Cholesterol, Heart Failure)
  6. Musculoskeletal (Spine Wellness)

Members can elect to receive the following support in every program: 

  1. New member “welcome call” outreach within 30 days of plan enrollment
  2. Telephonic coaching
  3. Support building a personalized health plan
  4. Help with scheduling appointments and preparing for visits with primary care physicians 
  5. Assistance accessing specialized resources, health care providers, information related to their condition and much more

Diabetes Program

The OhioHealthy Health Conditions Diabetes Program provides education, support, and access to nurses, health coaches, dietitians and licensed social workers. Potential benefits of participation include decreasing blood sugar parameters (individualized), preventing disease progression and improved medication understanding. This program is available free to members, 18 years and over.

You may elect to receive:

  • Diabetes education
  • Information and discussion points to review with your provider
  • Assistance with access to a dietitian
  • Support understanding the treatment plan the provider has outlined
  • Help connecting you to resources addressing behavioral health issues which may be impacting your physical health
  • An introduction to tobacco cessation

Weight Management Program

Successful weight loss means more than dropping pounds. Success includes having more energy, improving your overall health, and enjoying new healthy habits. The OhioHealthy Weight Management Program is personalized to your unique health journey and is free to all OhioHealthy members. 

 You may elect to receive: 

  • Assistance with accessing a dietician and medically supervised weight loss program 
  • Information and discussion points to review with your provider regarding surgical options 
  • Education and heart healthy information 
  • Help connecting you to resources addressing behavioral health issues which may be impacting your physical health

Chronic Obstructive Pulmonary Disease (COPD) Program

It’s time to breathe easier. Our COPD programs can help you manage and improve your condition. The OhioHealthy COPD Program, free to OhioHealthy members, is personalized to your individual needs. 

You may elect to receive: 

  • An introduction to tobacco cessation and pulmonary rehabilitation programs 
  • Support connecting to health care providers such as a pulmonologist 
  • Help understanding lung health and how to stay active as possible 

Chronic Kidney Disease Program (CKD)

OhioHealthy is committed to your kidney health, helping you manage and reduce progression of CKD. The OhioHealthy Chronic Kidney Disease Program is free to OhioHealthy members and personalized to you. You may elect to receive:

  • Referrals and information on programs proven to help individuals quit smoking
  • Assistance with access to a dietician and support with dietary changes
  • Assistance with understanding exercise programs designed for those with kidney conditions and support understanding the treatment plan the provider has outlined
  • Support medication adherence and treatment plan the provider has outlined
  • Education on disease basics including symptoms, the stages of CKD and additional support such as blood pressure and blood glucose monitoring

Cardiovascular Program  

A healthy heart is a happy heart. OhioHealthy is committed to your cardiovascular health, helping you manage and reduce risks like Hypertension (High Blood Pressure), which is a significant cause of heart disease, the leading cause of death in Ohio. That’s why understanding your risks is so important. The OhioHealthy Cardiovascular Program is free to OhioHealthy members and personalized to you. 

You may elect to receive: 

  • Referrals and information on programs proven to help individuals quit smoking 
  • Assistance with access to a dietician and medically supervised weight loss programs 
  • Information on heart-healthy exercise programs designed for those with cardiovascular conditions 
  • Taking medication for Hypertension? Health coaches can provide education on disease basics including symptoms and additional support such as blood pressure monitoring and tips to manage and prevent future complications/exacerbation
  • Support understanding the treatment plan the provider has outlined 

Musculoskeletal (Spine Wellness) Program 

A 5-week educational program designed to promote spine health and injury prevention to maintain a healthy spine long-term. During weekly interactive sessions, OhioHealth professionals will allow participants to gain a better understanding of the importance of core strength and mindful practices to maintain a healthy spine, long term.

*Sessions are held 4 times a year, in January, April, July and September

SPINE WELLNESS JOIN TODAY! 

  • Week 1: Pain interpretation and healthy biomechanics
  • Week 2: Mindfulness & stress reduction
  • Week 3: Mobility, flexibility and foam rolling
  • Week 4: Strength and stability (lumbar, core, cervical and thoracic)
  • Week 5: Strength training (free-weights and deadlifts)
